Community Health and Dental Care is a community health center that offers the Pottstown community a team of dedicated healthcare professionals who provide complete care for the whole family.

Formed through the funding of the Pottstown Area Health and Wellness Foundation along with various local businesses, CHDC opened its doors in Oct 2008 and saw 39 patients within the first week.

CHDC has since grown to serve thousands of medical and dental patients each year at each of its three Pottstown locations and recent expansion to Barto opening September 2020.

With a qualified and caring staff, CHDC provides a range of services from primary care and women’s health, to dental and vision. CHDC also offers immunization, pediatric care, physical therapy, podiatry care, and behavioral health services. As a recognized Center of Excellence for Opioid Use Disorder, CHDC also provides support with substance abuse.

As a non-profit community health center, CDHC’s role in the community is to provide affordable, accessible and quality healthcare to all. With a mission to identify and close the gaps in health services, CHDC ensures access to healthcare to everyone who needs it regardless of their ability to pay.

CHDC accepts most major Commercial, Medicare and Medicare supplement plans, and provides a health care discount for eligible uninsured and underinsured patients through annual funding from its Federal partner, Health Resources and Services Administration (HRSA).

The vision of CHDC is to be the health care provider and employer of choice by establishing a culture that champions patient centered-care, promotes staff engagement, embraces the latest technology, and pursues partnerships and strategic alliances.
