Jaisohn Center Opens New Community Wellness Plaza in North Wales

The Jaisohn Center has opened a new Community Wellness Plaza in North Wales, offering coordinated care for seniors and families.

A new community-centered health hub is now open in Montgomery County. The Philip Jaisohn Memorial Foundation has officially opened the Jaisohn Community Wellness Plaza in North Wales, creating a one-stop destination for health, wellness, and human services.

Built on more than 50 years of service, the new Plaza was designed to improve access to care and strengthen coordination for seniors and families. By housing multiple services in one trusted location, the center aims to reduce barriers while supporting older adults who want to age with dignity in their own community.

The Plaza currently offers primary care, home health and home care services, mental health counseling, health insurance assistance, community health navigation, and social services. Future plans include expanded offerings such as blood work, acupuncture, pharmacy services, pain management, and specialty care including ophthalmology and dermatology.

According to Jaisohn Center President George Choe, the Plaza represents a major step forward in how care is delivered locally, with an emphasis on accessibility, privacy, and culturally responsive services. The thoughtfully designed space is intended to be especially welcoming for older adults and first-time visitors alike.

Located at 1180 Welsh Road, the Jaisohn Community Wellness Plaza is positioned as a central point of connection for coordinated, community-based support in Montgomery County.
