Holy Redeemer Health System expands its digital presence

Holy Redeemer Health System, based in Huntingdon Valley in Montgomery County, announced it will expand its connection with Prepared Health, based in Chicago, and the company’s enTouch network.

Holy Redeemer has used the network for the last year — using the network system to allow caregivers and case managers with the ability to collaborate in real time. That provision eliminates the gaps that come when multiple providers are involved after a patient is discharged from the hospital, writes John George in the Philadelphia Business Journal.

Patricia O’Brien, senior vice president and chief administrative officer for Holy Redeemer Home Care & Hospice, said Prepared Health’s technology supports the health system’s commitment to safe transitions for hospice and home care patients and open, consistent communication with its referral sources and physician partners. “This technology allows our team to refer and accept patients, update our care partners on patient status, and work collaboratively to prevent unnecessary ER visits and hospital admissions,” O’Brien told the Philadelphia Business Journal
